Questionnaire

Here’s where you tell us all about you and your dog. This info, which we keep on file, will help us give them the most appropriate care. One form per dog please.

To ensure everyone’s stay is a happy and healthy experience, the following regulations apply. Please read and initial each clause. Just one form per household.

vaccinations

Email current vet records showing your dog has vaccinations for Rabies, Bordetella and DA2PP. Also, please apply a vet-recommended flea treatment before your dog’s visit.

Email paperwork

Before your dog’s first boarding, we’ll need to observe them on a daycare visit first.

what to bring

  • Food for their stay + a little extra (no huge bags or containers)

  • Any medications your dog may require + instructions

  • A well-fitted collar, but no harness or leash

  • Toys if you trust your dog with them

  • We can provide clean bedding and bowls
